Greg Marchand M.D. est un obstétricien et gynécologue américain connu pour ses procédures chirurgicales inventées[1],[2],[3]et ses records du monde chirurgical.[4],[5],[6],[7],[8],[9],[10],[11],[12] Sa technique pour retirer les trompes de Fallope au moment de la césarienne a été présentée à Paris en mars 2018 et publiée dans le Journal européen d’obstétrique et de gynécologie.[13] Marchand apparaît souvent comme consultant en obstétrique et gynécologie dans les médias du grand public[14],[15],[6],[16],[17].
Biographie[modifier]
Marchand est originaire de Johnston, Rhode Island, et a fait ses études de médecine à la Spartan Health Sciences University dans les Caraïbes.[18] Il a complété sa formation en obstétrique et gynécologie au University of Tennessee Health Science Center à Memphis, Tennessee[18].
Techniques chirurgicales inventées[modifier]
La technique chirurgicale "Marchand Salpingectomy" a été développée par Marchand peu après que l'American College of Obstetrics and Gynecology a publié l'avis n° 620 du Comité.[19] L'avis du comité a radicalement changé la chirurgie gynécologique en recommandant la salpingectomie (ablation des trompes de Fallope) dans les cas où une ligature des trompes aurait été recommandée auparavant.[19] Marchand a décrit et publié la technique au Congrès de clinique du 24 octobre 2017 du American College of Surgeons à San Diego, CA.[1],[2]
Le Dr Richard Demir et Marchand ont mis au point une technique sans nom pour le placement laparoscopique d'un cerclage cervical afin de prévenir l'insuffisance cervicale menant à une naissance prématurée. Une vidéo décrivant cette technique a ensuite été publiée lors du 32e Congrès international britannique des obstétriciens et gynécologues, à Belfast, au Royaume-Uni, le 20 juin 2010.[3],[20] Cette technique a été ajoutée à la liste publiée des compétences essentielles de la School of Medicine de l'université Rutgers en 2018[21].
Dossiers chirurgicaux[modifier]
Marchand et le Dr Richard Demir ont enlevé le plus large utérus jamais enlevé par hystérectomie laparoscopique en 2007.[22] L'utérus pesait 3 200 grammes et le record mondial a été vérifié et reconnu par Guinness World Records en 2008.[8] Un résumé décrivant la technique chirurgicale et les détails a été publié plus tard dans le numéro du 4e trimestre du Journal of the Society of Laparoendoscopic Surgeons de 2010;[23]; ce record a été battu en 2009 par le Dr Rakesh Sinha du BEAMS Hospital à Bombay, en Inde avec un utérus de 4100 grammes[24].
En 2015, Marchand a prélevé une masse ovarienne de 17 centimètres sur une patiente qui était initialement présumée bénigne.[6],[4],[5],[9] La pathologie peropératoire a montré que la masse était maligne, et le Dr Snehal Bhoola, gynécologue oncologue, a aidé Marchand à réaliser l'ensemble de la chirurgie du cancer de l'ovaire par voie laparoscopique. La combinaison de l'ablation de la très grande masse et de l'intervention de stadification complète a été la plus grande masse ovarienne à avoir jamais reçu une stadification chirurgicale par des moyens entièrement laparoscopiques. Ce record a été vérifié et reconnu par la World Record Academy en 2017.[9] Le record a fait l'objet d'importantes critiques parce que même si la masse n'était pas connue comme étant cancéreuse avant l'ablation, elle a été enlevée par morcellement laparoscopique, ce qui a augmenté le risque de propagation du cancer au niveau de l’abdomen[25],[26].
Marchand a réalisé en 2018 une hystérectomie laparoscopique à port unique par la plus petite incision jamais pratiquée, 11 mm.[11],[12] Ce record a été vérifié et reconnu par la World Record Academy en 2018. L'Académie a rapporté que le record précédent avait été détenu par la Cleveland Clinic et était de 15 mm[11].
Santé personnelle[modifier]
Marchand a été diagnostiqué en 2010 d’un choriocarcinome, un cancer testiculaire agressif.[4] Marchand a subi une chirurgie et des traitements cette année-là et rapporte qu'il est en rémission depuis cette date.[5] Marchand cite son combat personnel comme une inspiration pour créer des techniques chirurgicales favorable au traitement du cancer à travers ses entretiens et ses publications[7].
Morcellation[modifier]
Deux des dossiers de Marchand portent sur la morcellation, un processus chirurgical dont les études ont démontré le potentiel de propagation des cellules cancéreuses dans l'ensemble du corps du patient.[25] Marchand a fait l'objet de nombreuses critiques après son deuxième record mondial en 2017, car ce record, contrairement au premier, a été établi après que la FDA a émis en 2014 un avertissement sur la morcellation[27]
